Please see the link below to our online intake form for you to fill out as best you can and submit so that we can then determine if and how we can help you. This will take about 15 minutes and the more detail you enter the more effectively we can design a treatment plan for you.

Click here or copy and paste: https://form.jotform.com/202918502821047 Your initial appointment at Ford Acupuncture will consist of a detailed discussion about your past and current health, your signs and symptoms, a pulse reading, tongue diagnosis and an overall differential diagnosis for you specifically, as well as your expectations around Acupuncture treatment. This does not include a treatment, it is an initial exam. If you would like a treatment on the same day, this can often be arranged and our fee for Acupuncture is $90/session. Should you require further additional help such as Cupping therapy or Herbs, these will be discussed and a seperate charge will be incurred.

To ensure your comfort during your acupuncture session, please ensure to either wear or bring to change into, appropriate clothing that allows access to shoulders and above knees (eg. singlet and shorts). We recommend that all patients eat some food at least 2 hours prior to every acupuncture appointment. Private health insurance rebates are available, so check with your health fund if you qualify for these as each policy is different. We understand that life happens!

If you need to cancel your appointment, you can contact the clinic; however, please keep in mind that we are a small business and cancellations within 24 hours of your appointment will often not be enough time to fill the spot so please be generous and thoughtful and only cancel if really necessary Please let us know if you have any questions or concerns and please contact us today.
